Player registration form

To register your child for the 2024 / 2025 season, please complete the following registration form and ensure you agree to the codes of conducts and terms and conditions.

All information will be held in confidence and will not be shared without written permission from the player or parent. Any information retained by the club will be used only to meet the requirements of the Football Association. Any data held will comply with Data Protection Act (GDPR).

To accompany each players registration, we require an annual signing on fee of £35.00, which is payable online via PayPal.

Medical conditions

If there is any medical condition that your coach or manager should be aware of, please let us know about it. Medical conditions will not prevent your child from playing for the club unless the team manager, coach or club physio deems otherwise.

Members of WTYS are not permitted to administer medication. If routine medication is required during training, a match or a tournament, we ask that parents or carers undertake this or give written permission for manager/coach to administer.

If your child requires first aid treatment for an injury, this will be administered by a trained manager or coach. In order to administer appropriate treatment, items of clothing may need to be removed. By submitting this form, you are giving your permission for this to happen should the need arise.

Codes of conduct


When playing football, I will:

  • Always play my best for the benefit of the team
  • Play fairly and be friendly
  • Play by the rules and respect the Referee
  • Shake hands with the other team - win or lose
  • Listen carefully to what my coach tells me
  • Understand that a coach has to do what's best for the team
  • Talk to someone I trust or the club welfare officer if I'm unhappy about anything at my club
  • Encourage my team mates
  • Respect the facilities home & away

I understand that if I do not follow the code, I may:

  • Be asked to apologise to whoever l've upset
  • Receive a formal warning
  • Be dropped, substituted or suspended from training

Parents and carers

I will:

  • Have fun; it’s what we’re all here for!
  • Celebrate effort and good play from both sides
  • Always respect the Referee and coaches and encourage players to do the same
  • Stay behind the touchline and within the Designated Spectators’ Area (where provided)
  • When players make mistakes, offer them encouragement to try again next time
  • Never engage in, or tolerate offensive, insulting or abusive language or behaviour
  • I will make myself familiar with safeguarding practices & review guidance on physical contact (5.6) & Acceptable Behaviours When Working With Young People (5.7) documents on the safeguarding section of

I understand that if I do not follow the code, I may be:

  • Issued with a verbal warning or asked to leave
  • Required to meet with the club committee, league or CFA Welfare Officer
  • Obliged to undertake an FA education course
  • Requested not to attend future games, be suspended or have my membership removed
  • Required to leave the club along with any dependents and/or issued a fine

Terms and conditions

Subscriptions will be charged at £25.00 per month, payable for 11 months beginning on 1st September 2024, through to and including 1st July 2025.

In addition to subs, any fines received by players for Yellow & Red cards, and/or any disciplinary charges, will be invoiced directly to the parent/carer of the player concerned for payment.

Should a player decide to leave once the season has started, registration fees are non-refundable, however subs payments will no longer be required from the following month after departure.

All playing kit and jackets remain the property of WTFC Youth Section and must be returned to their team Manager should a player leave.

Summer tournament entry fees are not covered by subs and are to be paid directly to the team manager responsible for booking.

In instances of genuine hardship, please discuss with the club to enable us to agree upon a suitable arrangement for paying subs.
